The choice between the anterosuperior and deltopectoral depends primarily on the experience of the surgeon as results has been consistently good in literature for both options; secondarily, it is guided by the indication and analysis of the advantages and disadvantages with both techniques. The main advantages for the trans deltoid approach are: simplicity, ease of axial preparation of the humerus, frontal exposure of the glenoid, lower risk for postoperative instability and potential preservation of the subscapularis tendon. Its drawbacks are the risk of weakening the anterior deltoid (mechanical or neurologic by damage to the distal branches of axillary nerve) and intact supraspinatus tendon; inaccurate positioning (excess height and superior tilt) of the glenoid component is becoming less of a problem when adequate release of the inferior capsule is performed.
