The highlights of this surgical techniques are the following: 1) The neutral alignment "dogma" in TKA has been recently challenged; 2) A slight, medic-lateral gap asymmetry reflects the natural knee kinematics; 3) The initial determination of the flexion gap is recommended in robot-assisted TKA; 4) TKA final alignment should be evaluated only after correct knee balancing, not as the initial goal.
