In our hospital all patients under 50 years of age are treated with primary BIG of the acetabulum. Our main philosophy is to restore the center of rotation of the hip joint and to increase acetabular bone stock in these young patients. A mesh is used in case the native acetabulum does not provide enough cover for the acetabular component. The mesh should lie on the bone and has to be fixated with a minimum of 5 screws. For our bone graft we chips 8-10 mm in diameter made from the patient’s own femoral head or an allograft fresh frozen femoral head, fresh frozen croutons can also be used.
