read more ↘ instability. This method does not require an extensive subacromial decompression or switching to different camera lenses to perform the procedure. This method describes placing knotless barbed anchors posteriorly. Alternatively, if there is any concern of compromised bone in this location, it is recommended to use larger screw-in anchors for the Remplissage procedure. The same general steps provided in this video are utilized, however, the larger screw-in anchors would be placed over a metal sled instead of through the drill guide used in this video. Similarly, if the surgeon prefers, the sutures can be secured between the two anchors with arthroscopic knot-tying fixation instead of using a knotless sliding mechanism. This may be a preferred fixation construct if there is decreased bone quality or concern with passing the sutures through a knotless anchor mechanism. Shoulder instability is a complex problem, especially in younger and more active patients. The Remplissage procedure can be beneficial and improve outcomes in certain patients with instability.
