read more ↘ abducted external rotation (positive hornblower’s sign), isolated teres minor atrophy on MRI or US, and/or isolated teres minor involvement on EMG. It is distinct from quadrilateral space syndrome in that it occurs with no involvement of the deltoid on exam or imaging. It also presents without paresthesias of the upper extremity or point tenderness of the area overlying the quadrilateral space as is classic in quadrilateral space syndrome. Teres minor atrophy results from damage to the primary nerve to the teres minor (PNTM) while QSS is a consequence of damage to the axillary nerve and/or transient blockage to the posterior humeral circumflex artery as they pass through the quadrilateral space.
This video demonstrates our technique for surgical release of the nerve to teres minor to relieve pain and improve function in isolated teres minor atrophy syndrome.
